My Bridge 4 Life

PCF has partnered with My Bridge 4 Life to launch a patient and caregiver support network that allows an individual to track their medical diagnosis, set goals, share Tips 4 Life, communicate with loved ones and print a personalized bridge book through four phases of illness: diagnosis, treatment, healing and wellness.

User Benefits:

  • My Bridge Plan: Your four phase (see below) progress plan.
  • My Blog: This is your public blog or journal and a place to engage and share your story with the Bridge 4 Life community, family, and friends. Blogs can be found by searching for key words or tags and you can access and comment on any blog by clicking on its title. You can also subscribe to other members’ blogs with the link below their picture.
  • My Community: Invite members, view their names and pictures and send emails to your community members.
  • My Medical: Your private, protected medical information can be uploaded to track contact information of doctors, nurses, physical therapists, homeopaths, etc. You can keep an organized record of any other important information, such as post-appointment summaries, details about medications and dosages, or advice from your medical professionals.
  • Tips 4 Life: Help people who will be following in your footsteps by providing useful tips and techniques. Each tip will be reviewed by our staff before being posted.

Phases:

  • Diagnosis: Begins with your Bridge Plan. This phase ends when the decision is made on the first course of treatment.
  • Treatment: Ends with the completion of the initial treatment. You are moving forward, be sure to engage your Community. Add details about your health journey including medical and treatment information.
  • Healing: begins when you set goals, then share and validate those goals and your target completion dates with your Community. Share Tips 4 Life that got your through treatment and illness with other users who can follow in your footsteps.
  • Wellbeing: Begins when you achieve your three goals. Celebrate! Create another round of goals as the foundation of your health plan for life.
